Form I-485, Application to Register Permanent Residence or Adjust Status.
Form I-864, Affidavit of Support, is a declaration of financial support from a sponsor.
Form I-765, Application for Employment Authorization, if you want to apply for a work permit while your adjustment of status is pending.
Form I-131, Application for Travel Document, if you want to travel outside the United States while your adjustment of status is pending.
A copy of your birth certificate, passport, and any other immigration documents you have.
Two passport-style photos.
